Aneurin Bevan University Health Board reported the incidents to the Welsh Government and the Human Tissue Authority once identified and has undertaken an investigation into its causes to prevent re-occurrence. The incorrect body releases happened in the same week in November 2023 and the health board has taken steps to address this issue and prevent future incidents.
