To propose that the Senedd:

1. Notes:

a) that Betsi Cadwaladr University Health Board has been placed back in special measures;

b) that independent reports have highlighted poor performance, leadership and culture amongst the executive team;

c) that while the Welsh Government's health Minister demanded the resignation of all independent board members, the executive team remain in post;

d) the failures of the health board to make significant improvements whilst in special measures and/or targeted intervention since 2015;

e) a Senedd petition stating that the health board has failed the people of north Wales and should be disbanded into smaller units is currently collecting signatures.

2. Calls for:

a) an independent public inquiry to consider:

i) the reasons for prolonged failings in the NHS in north Wales;

ii) why progress has not been achieved in spite of Welsh Government intervention;

b) a plan to map out a new start for the NHS in north Wales, including disbanding BCUHB and moving to a model of smaller health boards.
